School Safety and Discipline Report

The School Safety and Discipline Report (SSDR) is available for districts and schools to complete. This two-part report collects information regarding drug and violence related incidents and disciplinary actions occurring during the 2003-2004 school year as required under state and federal regulations. The incident form must be completed each time a violence or drug related incident occurs on school property. The discipline form must be completed each time a student is suspended for a violence or drug related incident, or is excluded for more than 10 school days, regardless of the reason. In addition, the discipline form must be completed each time a student with disabilities is suspended or removed to an alternative setting, regardless of the reason. A notification that this form would be available during the school year was in the October 6, 2003 Commissioner's Update .
The application is available through the Department's security portal at: www4.doemass.org. In order to access the form an individual must have the SSDR security role in Directory Administration.
