FY08 Academic Support Summer Enhancement Grant - Fund Code 625-B

End of Program Product and Evaluation Requirements for Summer 2009 Programs:

  1. Product Requirements

As noted in the FY09 FC 625-B RFP, all funded entities must develop and share electronically with the Department of Elementary and Secondary Education (Department) a product (a learning module or project outline that other districts can look at and learn from) by September 30, 2009.

Products were expected to be piloted during the summer of 2009 (July 1 - August 31, 2009) in the applicant's and/or in a partner's district, school, or collaborative. Refined versions are expected to be used during FY2010 Academic Support Programming (September 1, 2009 - August 31, 2010).

A sample template for a learning module Download PDF Document  Download MS WORD Document and a CSL project outline Download PDF Document  Download MS WORD Document are provided here for those who find it helpful. You may also create your own version of the products, as long as they meet the above described requirements.

Some of the submitted products may be posted to our website to serve as resources for other districts/programs. They may also be used in future Academic Support technical assistance sessions.

  2. Evaluation Requirements

    1. All 625-B grantees must submit individual student data information on students involved in the piloting of the proposed product. Data must be submitted through the Department's Security Portal's Academic Support Student Data application (use 625-B under "Grant Fund") by September 30, 2009. Directions are basically the same as for Fund Code 632/625 and are available via: http://www.doe.mass.edu/as/data/.

    2. Additionally, grantees must also fill out the FY09 625-B evaluation form Download PDF Document  Download MS WORD Document, which is due via email to AcSupport@doe.mass.edu also by September 30, 2009.
