How to request a hearing: A due process hearing may only be requested by filing a written request with the opposing party (ies) and simultaneously with the BSEA, at 75 Pleasant Street, Malden, MA 02148-4906, or by fax to the BSEA at 781-338-3398. A BSEA Hearing Form may (but is not required to) be used for this purpose
