Fiscal Year 2010 Connecting Activities Reporting Requirements

Connecting Activities Quarterly Reports should include:

1. The Narrative (not more than ten pages) to address:

   • Work-based learning experiences not accounted for in the Student/Employer Work and Learning Performance Report (WBL Levels A, B, C) i.e., job shadows, field trips, rotations, worksite supervisor trainings, teacher externships, etc.

   • A description of how your Connecting Activities and school-to-career professional development plans have been integrated and support each other

   • A Connecting Activities anecdotal "highlight" for the quarter.

   • Challenges you have overcome and challenges you continue to face this quarter.

2. The Student/Employer Work and Learning Performance Report generated through the online Work-Based Learning Plan Database or the pc-based STC Database.

3. The Fiscal Reporting Form

   *For legislative purposes, the grant period for this cycle of Connecting Activities funds is July 1, 2009 through June 30, 2010. Numbers for qualifying wages will be calculated based on all employer paid jobs/internships during this time period.
