West Springfield's Directory of Evidence Form
Template used by educators and evaluators to develop a "list" of anticipated evidence discussed during the goal-setting and plan development step of the 5-Step Cycle. The template identifies the Standard and allows the educator and evaluator to write in the Indicator and provide a bulleted list and a brief description of the artifacts that will be collected over the course of the evaluation cycle.
