Grants and Other Financial Assistance Programs: FY2012

Continuing Supplementary Support to Specific Underperforming School Districts: Holyoke Public Schools Improvement Planning and Implementation
Fund Code: 220-H

Purpose: The purpose of this state-funded grant program is to provide continuing supplementary support to Holyoke Public Schools in building leadership capacity at the district and school level, enhancing data systems used to inform decision-making, and supporting the district's literacy initiative.
Priorities: Holyoke Public Schools will focus supplemental efforts on supporting the District Management Council (DMC) team that is currently working with Holyoke on the topics listed above. This will include covering costs associated with the work of the team in the likelihood it is not covered by existing funds.
Eligibility: Only Holyoke is eligible at this time.
Funding: Funding is available for up to $212,700.
Fund Use: Funds can be used for activities and initiatives consistent with Department agreements with Holyoke regarding the work of the District Management Council.

Proposals should be submitted by 5:00 p.m. on the date due.

    Note: Included in the Workbook are the:
    1. Part I - General - Program Unit Signature Page - Standard Contract Form and Application for Program Grants; and
    2. Part II - Budget Detail Pages, including Form AM1 for future use.
Submission Instructions: Submit 2 hard copy sets with an original signature of the Superintendent.
