Reminder on Record Keeping Procedures with Preservice Performance Assessment (PPA)

All state-approved sponsors of teacher preparation programs are advised that the Department issues the following guidance in relation to record keeping for the Preservice Performance Assessment (PPA): The Preservice Performance Assessment (PPA) and its associated forms—including the cover sheet and the subject matter area evaluation questions—are required elements in the implementation of all state-approved teacher preparation programs leading to the Initial license, including the teacher specialist licenses. The PPA and all associated forms should be completed according to Department guidelines and retained on file in the candidate folder. The organization should not forward the PPA or any of its component parts to the Department. To safeguard the privacy and security of licensure candidates, the Department encourages sponsoring organizations to abbreviate social security numbers as recorded on the PPA, its associated forms, and on any other documents that are maintained on file in the candidate folder.
