Individual Non-Public School Report

The Fall 2010 Individual Non-Public School Report has been launched on September 27, 2010 and will once again be available for you to complete using a web-based form. Each year the information you provide is used for a range of important purposes both at the state and national levels, including federal grant allocations. This year we are requesting an additional piece of information related to the Commonwealth's anti hazing law. Please complete this paper form identified as "Table 8" by responding to the school level question and then mail or fax (781-338-6850) it back to the Department. We strongly encourage you to complete the online version of the Individual Non-Public School Report. It is a user friendly application and a much more efficient and timely way to provide the Department with necessary information. Please remember whether you are doing the report online or on paper that Table 8 is available only on paper . If you accessed the online form last year, your username and password will allow you access again this year. If this is your first time accessing the report online or if you have forgotten your username/password, please send an email to PrivateSchools@doe.mass.edu with the following information:
  • School code
  • School name
  • City/Town of school's location
  • Contact information for individual completing the report (First Name, Last Name, email address and phone number)
You will receive a reply email containing detailed instructions for completing the online form. To complete the paper version of the report, you may download the form at the following link: http://www.doe.mass.edu/infoservices/data/schedule.html. Please return the completed form to:

The online form or the paper report, and the supplemental sheet labeled as Table 8, are due to the Department by November 13, 2010. Upon completion of the online form or the paper report, please send a copy to your local Superintendent's office.
