The Fall 2012 Individual Non-Public School Report will be available for you to complete using a web-based form. Each year the information you provide is used for a range of important purposes both at the state and national levels, including federal grant allocations.
We strongly encourage you to complete the online version of the Individual Non-Public School Report. It is a user friendly application and a much more efficient and timely way to provide the Department with necessary information. If you accessed the online form last year, your username and password will allow you access again this year. If this is your first time accessing the report online or if you have forgotten your username/password, please send an email to PrivateSchools@doe.mass.edu with the following information:
- School code
- School name
- City/Town of school's location
- Contact information for individual completing the report (First Name, Last Name, email address and phone number)
You will receive a reply email containing detailed instructions for completing the online form.
To complete the paper version of the report, you may download the form at the following link: http://www.doe.mass.edu/infoservices/data/schedule.html. Please return the completed form to:

The online form or the paper report is due to the Department by November 15, 2012. Upon completion of the online form or the paper report, please send a copy to your local Superintendent's office. If you need assistance, please call the Data Collection office at 781-338-DATA (3282).
