Surety Forms

Schools are required to provide surety in an amount set by the Office of the State Auditor to satisfy the indemnification requirement of Massachusetts General Laws Chapter 75C, § 4; or 75D, § 14; or 93, § 21F; whichever is appropriate. Schools may use the form provided by its bank or insurance company, or, schools may use one of the forms provided below. If working with a bank, please also provide them with the corresponding instruction sheet.
