CSPD Training Project Module One: The Massachusetts IEP Process
Introduction - Transition on the IEP
The goal of this unit is to highlight key school to post-school transition issues, emphasizing the integrated approach to planning for adult living, and to alert workshop participants to the responsibility of the IEP Team to document pertinent transition information on the IEP.

You will need to provide multiple copies of IEPs (perhaps 6-10 IEPs) in order to do the Transition Exercise.
This unit should be presented in the following order:
Transition on the IEP PowerPoint Presentation - As you introduce this topic, please be clear that this is an overview of the issues surrounding transition, and a push to have an integrated, rich, and creative discussion about transitional planning documented on the IEP. Transition Exercise I  Divide up into small groups. Each participant needs a copy of the IEPs and the Transition Exercise Template. All working off the same IEP, complete the template. Repeat this with multiple IEPs (time permitting). Look for trends, areas of strengths and weaknesses. In the large group, collect information on trends, areas of strengths and weaknesses - see if you can draw some conclusions.
Transition Exercise II  In this exercise each group has the opportunity to be pro-active and address Tammy's transition issues and needs. BE CREATIVE. A large group discussion will give the workgroup participants an opportunity to hear what others have considered.
