Module Title:

Transition From Adolescence Into Adulthood in Massachusetts Help for Viewing PowerPoint Presentation

Module Goal: Introduction of the issues related to youth with disabilities as they transition into adult life.

Module Objectives:

• To increase awareness of the availability of transition services for youth with disabilities.
• To demonstrate how issues related to transition are integrated into the Individualized Education Program (IEP).
• To highlight the need for the Special Education Team to be involved in providing transition services.
• To improve interagency collaboration for adolescents with disabilities.
• To review the process of filing a Chapter 688 referral.

Intended Audience: New Team Chairpersons, Special Education Teachers, Paraprofessionals, General Educators, and Related Providers.

Training length: 1-2 hours
