*Students receiving residential, day, or vocational DMH children's services must meet eligibility requirements for DMH adult case management.
**Mentally retarded or mentally ill students in need of residential services should not be referred to MRC for Transitional Planning. Those students with a severe physical disability, in need of residential services, should be referred to MRC. (Eligibility for the Independent Living Program will be determined.)
In the event it cannot be clearly determined which agency is appropriate, send the 688 referral and complete referral package to:
