Cohort Sample Activity Overview for the 2016-2017 School Year:

Cohort 1
  • Spring 2017, districts will collect and report exit data for preschool students with IEPs in order to measure their social-emotional, academic, and behavioral growth (Indicator 7B).
  • During school year 2016-2017, districts will work with the ESE to collect district level data through a survey measuring the percent of parents with a child receiving special education services that report that schools facilitated parent involvement as a means of improving services and results for children with disabilities (Indicator 8).
  • Spring 2017, districts will conduct a self-assessment and report data to measure compliance with early childhood special education transition: the percent of children referred by Early Intervention (Part C) prior to age 3, who are found eligible for special education (Part B) services, and who have an IEP developed and implemented by their third birthdays (Indicator 12).
Cohort 2
  • Fall 2016 - Complete Special Education Program Plan Statement - Due October 1, 2016.
  • Spring 2017, districts will collect and report exit data for preschool students with IEPs in order to measure their social-emotional, academic, and behavioral growth (Indicator 7B).
  • Spring 2017, districts will conduct a self-assessment and report data to determine compliance with secondary transition requirements: the percent of youth aged 16 and above with transition planning that includes appropriate measurable postsecondary goals based upon age appropriate transition assessment, transition services that will reasonably enable the student to meet his/her post-secondary goals, and annual IEP goals related to the student's transition services needs (Indicator 13).
Cohort 3
  • Spring 2017, districts will conduct a self-assessment and report data to measure the percent of children with parental consent for initial evaluation, who were evaluated and eligibility determined within 45 days (Indicator 11).
  • Spring 2017, districts will collect contact information from 2016-2017 exiters with IEPs in preparation for the Spring 2018 14B data collection. There is no ESE reporting requirement for 14A (Indicator 14A).
Cohort 4
  • Fall 2016 and Spring 2017, districts will collect and report entry and exit data for preschool students with IEPs in order to measure their social-emotional, academic, and behavioral growth (Indicators 7A and 7B).
  • Spring/Summer 2017, districts will administer a post-school outcomes survey to all students with IEPs who exited school the previous year and report data to ESE (Indicator 14B).
