DESE has revised the Extended Evaluation form (EE 1-EE 2) to conform to Administrative Advisory SPED 2019-2: Extended Evaluations, issued on April 1, 2019. As described therein and in 603 CMR 28.05(2)(b), the purpose of an extended evaluation is to gather additional information to enable the Team to write a full IEP for the student.
Special Education IEP Forms and Notices are available on the DESE website; including the Extended Evaluation form (EE-1, EE-2) in English, Albanian, Arabic, Cape Verdean, Chinese, French, Haitian Creole, Hmong, Japanese, Khmer, Korean, Laotian, Portuguese, Russian, Somali, Spanish and Vietnamese.
