IEP Evaluation Consent Form Attachment N1A, Revised

DESE has revised the IEP Evaluation Consent Form Attachment (N1A) to include a checkbox for parents to indicate their request for evaluation summaries.

Please use the updated form and comply, if so indicated, with the parents' request. A translated version of the form updated with the new checkbox for parents will be available soon.
