New PL1, PL2, and PL3 Forms
Beginning in the 2007-08 school year, SIMS codes regarding special education placements/educational environments have changed. In the past, DOE034 has documented educational environments for all students. Now DOE034 is the SIMS field for educational environments for students who are 6-21 years of age and the new DOE032 is educational environments for children who are 3-5 years of age. Due to these changes in SIMS codes the PL1 and PL2 forms have been revised accordingly. They now coincide with the new DOE032 and DOE034 codes.
The new forms are split by age. Three through five year olds have their own PL1 and PL2 forms and 6-21 year olds have different PL1 and PL2 forms. In addition there is a new PL3 form for all students receiving special education services, ages 3-21.
Placement Consent Form - PL1: 3-5 year olds
This form is to identify and document parent's consent to placement for children who are 3-5 years old and receiving special education services. PL1 (3-5)
Educational Environment / Placement - PL2: 3-5 year olds
This two-page form is for school or district record keeping only. It provides a flow chart and excel spreadsheet for schools or districts to assist in determining the appropriate DOE032 code for educational environment for 3-5 year olds. PL2 (3-5)
Placement Consent Form - PL1: 6-21 year olds
This form is documentation of consent to placement for students 6-21 years old and receiving special education services. Parents of the student, or the student if they are 18 years or older, as appropriate, identify and document consent to the student's placement. PL1 (6-21)
Educational Environment / Placement - PL2: 6-21 year olds
This form is for school or district record keeping only. It provides case studies to assist schools or districts in determining the appropriate DOE034 codes for educational environments for 6-21 year olds. PL2 (6-21)
Primary Disability / Level of Need - PL 3: 3-21 year olds
This form is for school or district record keeping only. It is used to document the student's primary disability (DOE036) and level of need (DOE038). This form is to be used for students 3-21 years old. PL3
