New Chapter 688 Referral Form

The Chapter 688 Referral form has been revised. The new referral form Download PDF Document  Download MS WORD Document is available on the ESE website for district use. School systems must refer all students with severe disabilities, who may be eligible for adult services, through the Chapter 688 referral process. For this reason, use of the new referral form is mandatory and should begin immediately. All previous referral forms should be discarded. I continue to encourage districts to use "A Guide to Chapter 688: Massachusetts' Transitional Planning" brochure Download PDF Document  Download MS WORD Document and the Chapter 688 page on the Department's website to aid in Team decision making and the referral process.
