Federal Regulations, which govern and provide funding for costs associated with the National School Lunch, School Breakfast, After School Snack, Special Milk, and Food Distribution Programs, require an annual renewal application be completed and submitted, by all public and private schools, and residential facilities participating in any child nutrition program, to the Massachusetts Department of Elementary and Secondary Education for review and approval. Once again, this year,
all renewal documents will only be available in electronic format on July 26, 2010
. In order to submit these forms online you must first obtain a password from your school's district Technology Coordinator. You may already have this if you have accessed Department information in the past. The renewal forms are accessible through the Department's website,
www.doe.mass.edu
. Select the
Security Portal
from the drop box in the upper right side of the main page, and click the orange arrow. After entering the username and password, please select the following:

School Lunch and Breakfast
Applications
App Packet
This will bring you to your district's renewal packet for 2011 school year.
Please make sure the amendment in the upper right hand corner is Amendment 1, 2011.
Click on each item to open and enter the correct information. When updating the
Authorized Signature section
of the application packet the following officials must be listed with email addresses: Superintendent or individual with legal and financial responsibility, business administrator, food service director and ESE Liaison. The
District Application
is the last item to be submitted, and represents submission of the entire packet. The instructions for completing the renewal application are available in the
Document and Reference Library
on the main page of the Nutrition School Lunch application in the Security Portal.
The renewal application process must be completed no later than September 24, 2010.
