Individual Non-Public School Report

I am pleased to announce that the Fall 2008 Individual Non-Public School Report will be available for you to complete using a web-based form. The paper version will continue to be available for download from our website and can be mailed in to the Department. Each year the information you provide is used for a range of important purposes both at the state and national levels, including federal grant allocations. To access the report online, please send an email to PrivateSchools@doe.mass.edu with the following information. This email account will be in place through the end of the calendar year to assist in the data collection process.
  • School code
  • School name
  • City/Town of school's location
  • Contact information for individual completing the report (First Name, Last Name, email address and phone number)
You will receive a reply email containing detailed instructions for completing the online form. To complete the paper report, you may download the form at the following link: http://www.doe.mass.edu/infoservices/data/schedule.html. Return the completed form to: Massachusetts Department of Elementary and Secondary Education Data Collection/3rd Floor 350 Main Street Malden, MA 02148-5023 The online form or the paper report are due in the Department by November 7, 2008.
