The Fall 2009 Individual Non-Public School Report will be launched on September 25, 2009 and will once again be available for you to complete using a web-based form. We strongly encourage you to complete the online version of the report. It is a user friendly application and a much more efficient and timely way to provide the Department with necessary information. The paper version will continue to be available for download from our website and can be mailed in to the Department. Each year the information you provide is used for a range of important purposes both at the state and national levels, including federal grant allocations.
If you accessed the online form last year, your username and password will allow you access again this year. If this is your first time accessing the report online, please send an email to PrivateSchools@doe.mass.edu with the following information:
- School code
- School name
- City/Town of school's location
- Contact information for individual completing the report (First Name, Last Name, email address and phone number)
You will receive a reply email containing detailed instructions for completing the online form.
To complete the paper report, you may download the form at the following link: http://www.doe.mass.edu/infoservices/data/schedule.html. Please return the completed form to:
