Directions for Use of IEP Forms and Notices



  1. The MS Word format of ED1, ADM1, EE1-EE2, CKL1-CKL5, IEP1-IEP8, PR1, PL1, PL2, PL3 and N3A were developed as on-line forms. We recommend the following procedure after opening each document:
    1. Click on Tools
    2. Click on Unprotected Document
    3. Add school district information
    4. Click on Tools again
    5. Click on Protected Document
    6. Click on Forms
    7. Click on 'ok'
    8. Save document to your MS Word file

You now have a template customized to your school district. When using for an individual student, if additional customization or additional pages are needed, use the same process.

  1. The MS Word N1, N1A, N2, and N3 documents were developed as examples of notices that contained the elements required by IDEA '97. They are 'Unprotected Documents' available to school district customization. If you choose to use these notices, we recommend the following procedure after opening each document:
    1. Add school district information
    2. Save document to your MS Word file

You now have a school district document that can be used for each student, as needed.

  1. Ch1 is not an on-line form and is 'unprotected'.
