Directions for Customizing Forms for your District

The Specific Learning Disability Eligibility forms were developed as on-line forms. We recommend the following procedure for customizing each form for your district.

1. Open the Word document and save it to your computer
2. Open your saved document
3. Click on Tools
4. Click on Unprotect Document
5. Add school district information
6. Click on Tools again
7. Click on Protect Document
8. Click on Forms
9. Click on 'ok'
10. Save document to your MS Word file
