This CACFP Center Eligibility Check Flowchart is from the Office for Food and Nutrition Programs. It helps determine if a program is eligible to participate in the Child and Adult Care Food Program (CACFP). The process starts by asking if the program is a qualifying center (e.g., childcare, afterschool, adult day care, Head Start, or homeless shelter).

If yes, the chart continues with questions:

  1. Is the program currently operating?
  2. Has it been operating for at least two weeks?
  3. Does it have a UEI number?
  4. For childcare centers: does the site have an active EEC license? For others: does it have a compliant health and fire inspection?
  5. Have any board members or principals been convicted of fraud, theft, or similar crimes in the past 7 years?
  6. Is the program or any principals on the USDA National Disqualified List?

If the answer to all is "Yes" and not disqualified, the program may be eligible and is directed to complete New Sponsor Training. If any question is answered "No," the program is deemed not eligible at this time. A green hexagon at the end reads: "Your program may be eligible to participate in the CACFP. Please complete the New Sponsor Training to start the application process. Click here to begin the application process!"
