The Massachusetts IEP Improvement Project provides updated forms and resources to support students, families, and districts in the IEP process. This page includes essential documents such as the revised Individualized Education Program (IEP) Form, amendment forms, parental notices, and consent forms for evaluations and placements. Additional resources, including discussion guides, quick reference guides (QRGs) on transition planning and English learners, and translations in multiple languages, ensure accessibility for all stakeholders. Administrative and technical guides are also available to assist districts in implementing the updated IEP process effectively.
