Module Goal: To improve transition planning for students with disabilities.
The Massachusetts Transition Planning Form was developed to be a working document that helps you harness relevant input in the four required areas of Course of Study, Employment, Community Experiences and Daily Living Skills and Needs, towards identifying the desired outcome you are seeking for the student and the action plan that will help the student get there.
Module Objectives:
Intended Audience:Parents, Special Education Directors, Team Chairpersons, General and Special Educators and Related Service Providers and other Team members.
