Please note that you may fill out the following form electronically or on paper. After you complete the complaint form, please submit it via email at email@example.com or by mail to Problem Resolution System Office, Massachusetts Department of Elementary and Secondary Education, 75 Pleasant Street, Malden, MA 02148-4906. If submitting via email, for the signature line, you may either type your name or insert your electronic signature, both of which will be considered the legal equivalent of your manual/handwritten signature.
Please call 781.338.3700 or email firstname.lastname@example.org to receive a complaint intake form in any language. PRS staff can also take your complaint over the phone in any language.
